This will gather all of the necessary information that we will need to create the perfect website for you and your agency.

Please note: while we do have a great deal of stock generic content for websites your site will need some personal touches to the content, so we ask that you provide as much information on the form as possible, so when the initial draft of the website is done, we will have plenty of information to work together and finalize the content on your website.
I know we all want a Lexus or Ferrari for a website, but that is far more than your agency will need to get a web presence. Our website package is a professionally designed 4 to 6-page website with all of the necessary essentials to help you get your agency a web presence and give you the ability to market yourself online.
- If requested we can set up an email address for your company. This is an additional cost on top of the domain & hosting so we will help you with the purchase of an email package to be associated with your website’s domain name
We have provided the Website Development Plan form on the next page for you to communicate your site needs with us. Don’t hesitate to ask questions along the way, we’re here to help you. If you find yourself stuck while filling out the form, just reach out and we can answer questions and aid you in whatever you need. While some of the form fields are ‘required’, if you do not have any content or ideas for that part of the form, you can put a hyphen or type NA in the field. This will let us know that it’s a section that you need help with and we can work with you to come up with some great info for that particular section based on our stock content.
Things that are not within the scope of our work:
- We do not set up Facebook, Twitter, or any other social media platforms for your agency. We can integrate them with your website, and add the links for you, but Social Media Development is not part of our service offerings.
- We do not register SEO for your agency. We will give the tools to you, (plugins, etc. as well as several links to websites outlining how to submit your website to search engines) but SEO submissions to Google & other search engines can not be done by us, as it requires you to create accounts with your personal/company information. If we create listings for your agency, it will be linked to our information here as Certified Homecare Consulting and will create problems in the future for you if you were to begin using services (such as Google AdWords).
- We DO NOT recommend using Fat Cow, Domains.com, or any other budget hosting companies due to the lack of support and issues we’ve had in the past. If you would like to use a different provider, PLEASE ask us for recommendations.

You must purchase both a domain name (.com, .net, or other types of domain name) as well as a WordPress or website hosting package. without both of these items, we can not build your website. The form on the following page has fields for you to provide all of your website hosting information to us.

We will gladly set up a shopping cart at GoDaddy with all of the products that you will need to move forward with this part of the process.

Our Website Development Plan is a tool to assist you in planning for your website as well as to develop a mutually agreed-on plan between you and your web developer. The more information you are able to provide to us, the better we can put together a specific style website for your agency. Website Development Plan Introduction Below you will find a thorough questionnaire of what you would like included in your website development process. There are places that I will need you to provide information for, and we will outline, explain, and in some cases, we will give you examples of what we are looking for. The more information that you can provide to us for your website development, the more personalized we can make your website for your specific agency. Recommended Items (non-essential, but always welcome)
Some of the things we recommend for personalizing your website are:
- Photos of you & your staff
- Photos of your office (interior, exterior, or both)
- Personal story of why you started (supply this information in a separate word document if you wish to add it)
- Any other materials that you feel would be beneficial to your agency’s website
These aren’t mandatory things, but we suggest them as some people like to put very personal touches on their websites.
Unsupported Items
While quite a few website hosting companies offer “Website Builder” packages, we, unfortunately, do not work with “website builders”, as they are very limited in what we can do for our clients. These exclusions include, but are not limited to:
- GoDaddy GoCentral Website Builder
- HostGator Website Builder
- Wix
- Squarespace
- Weebly
Who is your target audience?
Please provide a target audience. This could be described by age, gender, locality, etc., or a combination of these. Think about the type of information your customer is most interested in and what you are providing. Are they female or male audiences? What type of income will they have? Generally, where are they most like located? What information will be valuable to your target audience and customers? Are they interested in learning more about your business? Are they interested in how your services fit their needs? Look at how your company helps your clients.
Website Development Checklist
The following items are necessary for the development of your website. If any of these things have not been completed, it will delay the production of your website. Please note, that we do have generic content for websites here, it is very common information, so we look to every one of our clients to help and supply us with additional information that they wish to have on their websites. Home Page
This is the landing page of your website. It is the best practice to provide a summary of who you are and what services you offer. You must make your home page content interesting and engaging because it needs to grab your visitors’ attention in the first few seconds they are on the page. What to include:
- A brief explanation of who you are and what you do
- a short description or bulleted list of your services
- a brief explanation of how your services will benefit the visitor
Other things you might want to consider sharing or having on this page, would be:
- some quick info about a couple of your services
- some engaging context
- possible other points of interest
About Us
Page Here you will provide an overview of the history of your business and educate your visitor about how you are different from your competitors, and why they should choose your agency. What to include:
- An overview of the business including the history of the business
- Specific information about the business
- a personal bio and photo
- any awards or special recognition you have received
- your agency’s unique points of differentiation
Services Page
This page is where you will provide information about all of the services you offer. Start the page with a brief overview or summary of your services and then list them below. If you have a large number of services or a lot of information about each service, you may want to think about separating them into categories What to include:
- A summary of services offered
- a list of services with descriptions (brief descriptions are ok, but the more detailed, the better)
- the benefits the visitor will receive from the service
- how your services are different from the competitors’ services
Contact Page
This page is to provide your site visitors with all of the different ways they can get in touch with you.
- Office address
- Phone number
- Fax number
- E-Mail Address
- Social media profile web addresses: (if any)
- Other
- Directions: (we do include an interactive google map, but if there are specific instructions to reach the office, this is where you will provide that)

Optional Pages
FAQs (optional page)
The FAQ page is used to answer questions that your prospective and current clients ask you repeatedly. A FAQ page on the website will serve your visitors better and cut down on the general questions you receive, leaving potential/existing clients to ask more specific questions. Always positively pose the questions and, while you must always answer the questions honestly, you also want to make sure you keep them interested. Your answers should help persuade the visitor to take the next step or action to begin services with you.
What to include: Questions you are asked repeatedly, questions that always come up when speaking with prospects, questions that address (and solve) possible objections your prospects have, and questions that can help remove any barriers or perceptions prospects may have. (Please supply an additional word document with questions & answers if you would like to add a FAQ section)
CAREERS (optional page)
Describe what you are looking for in an employee and the positions for which you hire. You may list specific positions you are looking to hire for, and we will provide a link to open an email so they may submit a cover letter/resume via email. If you would like to use an online job application, we can help integrate that into the website for you.
We DO NOT build online job applications, as that is FAR beyond the scope of work that we offer, and also requires a great deal of security and screening 24 hours a day, because of the sensitive nature of the information that candidates would be supplying. (Please supply an additional word document with job listings & detailed descriptions of the open positions if you would like to add a careers section)
Please remember: we do have generic website content, but we ask that you provide as much information on the form as possible, so we can work together to finalize the content on your website!
